Thanks to government public health offices, you can download a blank PDF copy of the vaccination record and fill it out for yourself 🙂

Face plant. You’ve got to be kidding but there it is:

After a few months, they realized it was stupid of public health “experts” to be distributing blank vaccination cards. These were widely available from numerous government web sites through the spring of 2021.

If you search for keywords you can still find copies of these record cards on some government sites, or, as of September 2021, there are copies available in the Internet Archive.

Below is a JPEG image of the front of the card. Originally, these images appeared in numerous news stories. Consequently, there is no security around any of these blank vaccination records.

As of July September 2021, the U.S. still has no method of providing an authenticated, verified vaccination record. The lack of planning and a solution indicates the depth of incompetence in public health. This is inexcusable.

Using a printout to create a false vaccination card is against the law.

Update – as of late summer of 2021, the Federal government has had Federal law enforcement agencies “seizing” blank cards printed outside the country. What a waste of money – since anyone can print their own anyway. The right solution is to create an authenticated, verifiable record, as numerous other countries have done. That the US has not done this is incompetence on the part of state and Federal government agencies.

Reminder – if you can do so, please get vaccinated.

This post was updated in September 2021 to add the link to the Internet Archive copy, to add the “Update” at the end, and some general text editing to improve readability.
