Nearly half are from indoor air pollution:

Figures from 2016 reveal that outdoor air pollution caused 4.2 million deaths and household air pollution caused 3.8 million deaths, air pollution from cooking with polluting fuels and poor technologies is a critical issue in many low income countries.

The indoor deaths are mostly from the use of wood and charcoal fired stoves indoors. In a program run by the World Bank, open wood stoves will be replaced with biomass or “crop waste” stoves that are more efficient. Since they are more efficient than what they replace, they can be purchased by the jet set crowd seeking to buy carbon offsets (really).
